The theme this year is: "The Future of Human Error Management in Aviation". This has a unique format, not just because it is non-profit. We bring together competitors, customers, and professionals from around the world to share best practices.

The 5th annual Safety & Quality Summit will take place in the heart of downtown Vancouver at the Fairmont Hotel Vancouver from March 30 – April 1, 2009. The theme will be "The Future of Human Error Management in Aviation." CHC is pleased to announce that this year's Keynote Speaker will be Capt. Haynes, who in July 1989 was the Commander of United Flight 232—a flight and its subsequent landing, by all accounts, no one should have survived.

Capt. Haynes successfully landed United Airlines Flight 232, a DC-10 which had suffered catastrophic damage to its number two engine causing all three of its hydraulic control systems to fail. Combining training, communication, preparation, cooperation and the 12 elements of an SMS made what seemed to be a non-survivable accident turn out to be one of the greatest stories of human performance triumphs where 184 people survived. Capt. Haynes will be speaking about his experience, “Against all Odds,” the morning of Monday, March 30.

CHC is also eager to report that internally recognized experts, Dr. Scott Shappell and Dr. Doug Weigmann have agreed to speak at the Summit on Human Factors Analysis and Classification /  Human Factors Intervention Matrix (HFACS/HFIX), Fatigue Management and Safety Culture in Aviation.

The format of the 2009 S&Q Summit will be a bit different than in previous years. On Saturday, March 28th, Nigel Woolmer a retired CHC employee with over 50 years experience in aviation safety and training, will be conducting an optional Accident Investigation & Incident Analysis course. The course will run until Monday, March 30 and will break in the morning so attendees may hear the keynote Speaker, Capt. Al Haynes. The course will resume again after lunch and will finish that afternoon. After Capt. Haynes delivers his speech, the sessions for the Summit will kick off after the lunch break. In addition,  CHC will be hosting an optional symposium to discuss Flight Data Monitoring (FDM) on April 2nd.
